Transition from PCA to CFSS | DHS is in the process of replacing personal care assistance (PCA) and the Consumer Support Grant (CSG) with Community First Services and Supports (CFSS). Overview | This page describes how to complete the AHC1 screen for both PCA and CFSS. The AHC1 screen contains assessment information needed to calculate the units or dollars for which the person is eligible. A PCA or CFSS SA does not use all fields on the AHC1 screen. This page only describes fields relevant to a PCA or CFSS SA. | ||
Phone field (PHONE) | In the PHONE field, the county/tribal nation enters the phone number DHS should call with questions, if needed. | ||
Assessor name field (ASSESSOR NAME) | In the ASSESSOR NAME field, the county/tribal nation enters the last and first name of the assessor who completed the assessment. | ||
Assessment date field (ASSESSMENT DATE) | In the ASSESSMENT DATE field, the county/tribal nation enters the date the assessment was completed in MMDDYY format. | ||
Begin date field (BEGIN DATE) | The BEGIN DATE field on the AHC1 screen must match the AGMT START DT field on the ASA1 screen. An entry in either field auto-populates the other field when the county/tribal nation presses function key F9. If the dates do not match, MMIS posts an exception code the county/tribal nation must resolve. For information on how to determine the correct begin date, refer to the agreement start date (AGMT START DT) section of DSD MMIS Reference Guide – ASA1 screen for PCA/CFSS. | ||
End date field (END DATE) | The END DATE field on the AHC1 screen must match the AGMT END DT field on the ASA1 screen. An entry in either field auto-populates the other field when the county/tribal nation presses function key F9. If the dates do not match, MMIS posts an exception code the county/tribal nation must resolve. | ||
PCA or CFSS field (PC/CF) | In the PC/CF field, the county/tribal nation enters X to indicate that the person uses PCA or CFSS. | ||
PCA or CFSS supervision field (PC/CF SUPERVISION) | In the PC/CF SUPERVISION field, the county/tribal nation enters X to indicate if the person uses PCA or CFSS qualified professional (QP)/worker training and development. | ||
Diagnosis 1 field (DIAGNOSIS 1) | In the DIAGNOSIS 1 field, the county/tribal nation enters the International Classification of Diseases (ICD) code for the person’s primary diagnosis. Note: For a list of diagnosis codes, refer to Centers for Disease Control and Prevention – ICD-10. | ||
Diagnosis 2 and 3 fields (DIAGNOSIS 2 and DIAGNOSIS 3) | In the optional DIAGNOSIS 2 and DIAGNOSIS 3 fields, the county/tribal nation enters additional ICD codes for the person’s additional diagnoses when applicable. | ||
Home care rating field (HOME CARE RATING) | The HOME CARE RATING field lists the person’s home care rating. The county/tribal nation cannot edit this field. MMIS auto-populates this field based on entries on the AHC2 screen. | ||
Total time field (TOTAL TIME) | The TOTAL TIME field lists the person’s total minutes per day. The county/tribal nation cannot edit this field. For a person with the home care rating EN, MMIS leaves this field blank. For a person without the home care rating EN, MMIS auto-populates this field based on entries on the AHC2 screen. | ||
Navigation | The county/tribal nation presses a transmit key to advance to the AHC2 screen. | ||
