Overview

Counties/tribal nations use MnCHOICES to conduct initial and annual assessments. This page is no longer applicable if the county/tribal nation uses MnCHOICES. The county/tribal nation should follow guidance on CBSM – Assessment applicability and timelines.

In circumstances when the county/tribal nation completes screenings using legacy documents, follow the process outlined on this page.

Applicability

The county/tribal nation completes a full-team screening when the following situations occur:

  • · Person requests or is assessed to need services for the first time
  • · Person or legal representative requests a full-team screening
  • · Person needs an annual reassessment
  • · Changes in a person’s service needs change the person’s level of care and, therefore, require eligibility authorization from DHS
  • · Child with a current diagnosis of unspecified developmental disability reaches 5 years old.
  • Timeline

    The county/tribal nation must enter a DD screening document into MMIS within 90 days of the screening. The county/tribal nation will receive an MMIS system exception code if it does not follow the 90-day timeline.

    Process

    The screening team must convene and participate in the screening process when a full-team screening is required. For more information, refer to DD SD Codebook – Screening team responsibilities.

    Then, the county/tribal nation enters a DD screening document into MMIS with field 24 (i.e., action type) coded as 01.

    Screening documents are subject to MMIS system exception codes. For information about specific codes, refer to DD screening document exception codes (PDF).

    DHS responsibilities

    MMIS automatically routes screening documents that meet certain criteria to DHS. DHS must approve screening documents in certain circumstances. This review is required in many situations, but most commonly if the screening document includes certain:

  • · Diagnosis codes
  • · Level of care determinations
  • · Services.
  • DHS may request additional information to determine a person’s eligibility for funding. DHS reviews and approves screening documents within 10 working days of submission.
