Comparison of MHM and RSC-TCM

Moving Home Minnesota (MHM) and relocation service coordination – targeted case management (RSC-TCM) can both support a person on Medical Assistance (MA) as they move to the community. However, MHM and RSC-TCM have different eligibility requirements.

A person has the right to choose which program/service is right for them, but it can be hard to decide which option to use. This page provides a comparison of MHM and RSC-TCM. It can be useful to help a person make an informed choice about transition services.

Note: A person cannot use MHM or RSC-TCM if they currently reside in the community.

Length of stay requirement

Yes, a person must have been in a qualified institution for 60 or more days. They may start their qualified institutional stay at one institution and then move to another qualified institution. This time period must be continuous to fulfill the 60-day institutional stay requirement.

Assessment requirements and level of care

A person can meet the institutional level of care requirement through one of the following methods:

  • · MnCHOICES assessment.
  • · DD Screening Document, DHS-3067.
  • · LTC Screening Document, DHS-3427 (PDF).
  • For institution for mental disease (IMD)/substance use disorder (SUD), a person can meet the institutional level of care requirement through Professional Statement of Need (PSN), DHS-7122 (PDF).

    A person needs an assessment to determine and document their eligibility for RSC-TCM through the following methods:

  • · MnCHOICES assessment.
  • · DD Screening Document, DHS-3067.
  • · LTC Screening Document, DHS-3427 (PDF).
  • Limitations

    A person receives 180 days of MHM transition planning and coordination. DHS may grant extensions to the 180-day transition planning and transition coordination eligibility span under certain circumstances. A transition coordinator may request an extension by submitting MHM Communication Form, DHS-6759H (select the transition coordination extension request option from the reason for communication drop-down menu).

    A person receives 365/366 days of MHM demonstration and community services after they move to a qualified community residence.

    MHM community services will be suspended if the person is re-institutionalized for 30 days or more and then resume once the person returns to the community.

    A person receives 180 days of RSC-TCM.

    The 180-consecutive-day limit is for each admission to an eligible institution after a stay in the community.

    The use of any targeted case management service will begin the 180-day timeline for RSC-TCM.

    Qualified community residence requirements

    A person enrolled in MHM must transition to a qualified community residence to maintain their eligibility to receive MHM services in the community. A qualified community residence should honor personal choice and control of the person’s home and give them opportunities for independence and community integration.

    A qualified community residence includes:

  • · A home owned or leased by the person or their family member.
  • · An apartment with an individual lease and living areas in which the person or their family has control.
  • · A Minnesota Department of Health (MDH)-licensed assisted living residence with an individual lease, lockable access and egress and living areas in which the person or the person’s family has access or control.
  • · A home in a community-based residential setting (i.e., community residential settings [CRS], adult foster care or family foster care) in which no more than four unrelated people live.
  • Note: Sober living homes and environments, recovery residences and board and lodging are not considered qualified community residences.

    A person using RSC-TCM must transition to a community setting that meets the home and community-based services (HCBS) settings rule. For more information, refer to DHS – Minnesota’s implementation of the HCBS settings rule.
