MAIN screen

MMIS stores two types of screening documents for home and community-based services (HCBS) in the MMIS SCREENINGS file:

  • · Developmental Disabilities (DD) Screening Document (i.e., screening activities for the DD Waiver and OBRA nursing facility admissions for persons with developmental disabilities).
  • · Long-Term Care Screening Document (i.e., activities for HCBS waiver programs, nursing facility admissions and Special Needs Basic Care [SNBC]).
  • Workers with an appropriate MMIS security access may review or take specific action on screening documents:
    1. Tab to place the cursor in front of the SCREENINGS menu item located in the PRIOR AUTHORIZATION APPLICATIONS section on the MAIN MENU screen.
    2. Type the letter X to select the SCREENINGS file.
    3. Press a transmit key (i.e., the numeric enter key or control key) to advance to the ASCR screen.

    Enter a valid value in the ACTION CODE field on the ASCR screen:

  • · A (i.e., add) to create a new screening document.
  • · C (i.e., change) to change existing screening documents.
  • · I (i.e., inquiry) to review information only.
  • To review existing screening documents, complete the following fields:
    1. Type the letter I (i.e., inquiry) in the ACTION CODE field on the ASCR screen.
    2. Enter the recipient’s PMI in the RECIPIENT ID field. MMIS requires use of the PMI number to identify a person. Conduct a Recipient File search when the PMI number is unknown.
    3. Enter a valid value in the DOCUMENT TYPE field to review a specific type of screening document.

  • · Type a D to review DD screening documents.
  • · Type an L to review LTC screening documents.
  • · Leave the DOCUMENT TYPE field blank to review all screening types.
  • 4. Press a transmit key. If only one screening document exists for the PMI entered, MMIS opens the document and does not open the ASEL screen. If no history exists, MMIS displays a message at the bottom of the ASCR screen stating: NO DOCUMENTS MEET THE SELECTION CRITERIA.

    If there are existing screening documents for the PMI entered, the ASEL screen opens and lists existing screening documents for the PMI entered on the ASCR screen. MMIS displays the valid values (i.e., codes and dates) in fields that detail the recipient’s screening history.

    To review a list of screening documents and select a document to review:
    1. Use the tab key to place cursor in front of a document on the ASEL screen.
    2. Type the letter X in the SEL IND field to select the document.
    3. Press a transmit key to open the selected screening document.
    4. Press a transmit key to advance through the queue and review history.
    5. Press function key F6 to return to ASEL screen to select another document.
    6. Press function key F6 from the ASEL screen to return to the ASCR screen to begin entry of a new screening document.

    TYPE field valid values are D (DD Screening Document) and L (LTC Screening Document.

    DOCUMENT NUMBER field displays the document number MMIS assigned to the SD.

    PROVIDER NUMBER field displays the NPI (National Provider Index) or UMPI (Unique Minnesota Provide Index) number assigned to the case manager.

    PROVIDER NAME field displays the case manager’s name associated with the provider number in the PROVIDER NUMBER field.

    ACT TYPE field valid values are the action type codes indicating the action taken on a DD SD.

  • · 01 = FULL TEAM
  • · 02 = ANNUAL REVIEW
  • · 03 = SERVICE CHANGE
  • · 04 = WAIVER-IN
  • · 05 = WAIVER-OUT
  • · 06 = EXIT CHANGE COUNTY OF FINANCIAL RESPONSIBILITY
  • · 07 = EXIT RELOCATION OUT OF STATE
  • · 08 = EXIT DEATH
  • · 09 = EXIT NO LONGER FINANCIALLY ELIGIBLE
  • · 10 = EXIT OTHER
  • ACT TYPE field valid values are the activity type codes indicating the activity conducted on a LTC SD.

  • · 01 = TELEPHONE SCREEN
  • · 02 = PERSON TO PERSON ASSESSMENT
  • · 03 = VISIT/EARLY INTERVENTION
  • · 04 = RELOCATION/TRANSITION ASSESSMENT
  • · 05 = DOCUMENT CHANGE ONLY
  • · 06 = REASSESSMENT
  • · 07 = CASE MANAGEMENT/ADMINISTRATIVE ACTIVITY
  • · 08 = CAC/CADI/TBI REASSESS 65TH BIRTHDAY
  • START DATE field displays the start date of the screening document in MMDDYY format.

    ST field valid values are the codes indicating the status of the screening document.

  • · A = APPROVED
  • · S = SUSPENDED
  • · D = DENIED
  • · R = REPLACED
  • MAJ PROG field displays the major program of the recipient at the time of this screening.
