Introduction

The Developmental Disability (DD) Screening Document Codebook is a reference tool for lead agencies that enter DD screening documents into the Medicaid Management Information System (MMIS).

Once completed and authorized, the DD screening document records compliance with requirements of Minnesota's federally approved waiver plan and Medicaid state plan.

Reimbursement for Medical Assistance services (e.g., intermediate care facilities for persons with developmental disabilities [ICFs/DD], waiver services and nursing facility services) requires DHS' authorization of the DD screening document.

Manual revisions

The DHS Disability Services Division (DSD) updates and expands this manual as needed and announces changes on DD SD Codebook – What's new.

Briefcase resource documents

Briefcase resource documents are a resource for case managers/service coordinators. These documents provide tools to set up and complete the screening process, as well as resources to answer questions from people who receive services and their families.
