When to use the DHS-6791B (CSP/CSSP)

Use of the MnCHOICES Community Support Plan with the Coordinated Services and Support Plan DHS-6791B

The Community Support Plan

The Community Support Plan portion of DHS-6791B is a required support plan following all MnCHOICES assessments. The CSP sets the foundation on which certified assessors can indentify and capture supports and services in the Coordinated Services and Support Plan of the DHS-6791B.

MCSO Exception: For people age 65 or older, counties and tribal nations may opt to complete the Managed Care Organization (MCO) Collaborative Care Plan, in anticipation that the person will be enrolled in a managed-care program for which they provide care coordination, instead of the CSP and CSSP.

The Coordinated Services and Support Plan

Counties and tribal nations use the CSSP DHS-6791B following a MnCHOICES assessment, when the person opens to any of the following programs or services:

  • Community Access for Disability Inclusion (CADI), including use of the CDCS service
  • Community Alternative Care (CAC), including use of the CDCS service
  • Brain Injury (BI), including use of the CDCS service
  • Elderly Waiver (EW) (fee for service) without Customized Living (CL) services, including use of the CDCS service
  • Alternative Care (AC), including use of the CDCS service
  • Developmental Disability (DD) Waiver, including use of the CDCS service
  • Developmental Disabilities/Related Conditions case-management services (Rule 185)
  • Personal care assistance service (fee for service authorized through a Type B service agreement)
  • Consumer Support Grant
  • Essential Community Supports
  • Moving Home Minnesota, once discharged, up to one year

Counties and tribal nations may use the alternative plan for the DHS-6791B CSSP after a MnCHOICES assessment in the following conditions:

  • EW customized living, adult foster care, and residential care settings where a residential services tool is required. In these circumstances; the Print RS Plan, Print CSP and Print Rate Guide RS Tool worksheets printed in combination acts as the alternative to the MnCHOICES CSSP when the EW Services Authorized worksheet in the RS Tool has been completed.

A MnCHOICES CSSP is not required in the following situations:

  • When the person is acccessing private or non-publicly funded services/programs
  • When the person is receiving the Family Support Grant only, and no case management is provided
  • When the person is using Early Intervention, IDEA Part C federal programs that help serve developmentally delayed children (commonly referred to as "Birth to 3" services)
  • When the person, who lives in an institution, is using a relocation or transition program, such as relocation services coordination, Alternative Care conversion case mangement or Moving Home MInnesota transition support
  • When the person receives only the following types of targeted case management: mental health; child welfare; vulnerable adult/developmental disabilities (except when it is used to reimburse Developmental Disabilities/Related Conditions (Rule 185) case mangement)
