Pre-Waiver Reimagine individualized home supports

Effective Jan. 1, 2022, DHS deleted this page. This service changed to the new version of individualized home supports during the rolling implementation of Waiver Reimagine streamlined services. The old individualized home supports policy is no longer effective as of Jan. 1, 2022.
