Person-centered support planning resources for lead agencies

Supervisors, case managers and assessors can use the following resources to support development of person-centered practices. This guidance will help you develop support plans that meet Minnesota Department of Human Services person-centered requirements.

Person-Centered, Informed Choice and Transition Protocol

Description

The Person-Centered, Informed Choice and Transition Protocol, DHS-3825 (PDF) includes guidelines for support planners to use and apply person-centered practices as required by state and federal law. It includes:

  • The history behind Minnesota’s transition to a person-centered approach
  • A list of who must use the protocol
  • What protocol items the lead agency must document for the HCBS Lead Agency Review.

Online module: Foundations for Person-Centered Support Planning

The booklet What does person-centered mean for me?, DHS-6803 (PDF) is a tool for case managers to introduce person centered principles to the people they serve.  The booklet includes a worksheet at the end to help a person prepare for his/her planning meeting.
