Long Term Services and Supports (LTSS) notice of action and appeals process

Description	This course is designed to guide county and tribal nation case managers and assessors through the notice of action and appeals processes. Minnesota state law requires case managers and assessors provide people with a notice of action: after every assessment and reassessment using the form DHS-2828A for every denial, reduction or termination of a service using the form DHS-2828B To better support these processes, DHS has developed a set of self-paced training videos to help case managers and assessors.
Learning objectives	After this course, case managers and assessors will be able to: understand what is needed to provide proper notice navigate the appeal process clearly communicate the denial, termination and reduction to the people you support
Target audience	County/tribal nation case managers and assessors
Registration information	Register using TrainLink: you must have a unique key to register and receive credit for training. Go to TrainLink Select Disability Services Learning Center Select Sign On in the upper right hand corner Enter your unique key Select Find a Course Select All DSD Classes Search ‘LTSS notice of action” Locate the session Select the course Select Start Course to begin and enroll in the class.
