Self-paced online course:
Long Term Services and Supports (LTSS) notice of action and appeals process

Description

This course is designed to guide county and tribal nation case managers and assessors through the notice of action and appeals processes. Minnesota state law requires case managers and assessors provide people with a notice of action:

  • after every assessment and reassessment using the form DHS-2828A
  • for every denial, reduction or termination of a service using the form DHS-2828B

To better support these processes, DHS has developed a set of self-paced training videos to help case managers and assessors.

Learning objectives

After this course, case managers and assessors will be able to:

  • understand what is needed to provide proper notice
  • navigate the appeal process
  • clearly communicate the denial, termination and reduction to the people you support

County/tribal nation case managers and assessors
