This monthly webinar series focuses on different types of foundational knowledge and skills for effective support planning. This month’s focus is on consumer-directed community supports (CDCS).

DHS will collect questions and topic recommendations that arise during the sessions to create online, self-paced training modules about support planning after the series is complete. We also will provide the most up-to-date information, resources and initiatives about support planning.

This will include contributions from policy leads from the following DHS areas:

• Aging and Adult Services.
• Behavioral Health.
• Disability Services.

Learning objectives

At the end of this session, participants will be able to:

• Locate and use available CDCS resources.
• Describe self-directed services.
• Highlight pros and cons of CDCS to the people you work with so they can make an informed decision.

Target audience

New and experienced staff from lead agencies, tribal nations and contracted agencies who use the MnCHOICES Support Plan, including:

• Assessors.
• Care coordinators.
• Case managers.
• Lead workers and supervisors of those listed above.

Registration information

Register using TrainLink

You must have a unique key to register and receive credit for training. Note that the TrainLink system does not allow same-day registration.

Steps

• Go to the TrainLink homepage.
• In the middle of the page underneath “Learning Centers,” select Disability Services Learning Center.
• Sign On in the upper right corner.
• Enter your unique key.
• Select Upcoming Classes/Class Registration.
• Select All DSD Classes.
• Search for Developing a support plan.
• Select a session.
• Select Enroll in this class in the upper right, and follow the registration instructions.
