LTSS Improvement Tool lead agency feedback survey

On Feb. 6, 2024, DHS will email the LTSS Improvement Tool lead agency feedback survey to representatives chosen to receive LTSS Improvement Tool data extracts. This survey gives lead agencies an opportunity to share feedback that will inform changes to the LTSS Improvement Tool and the data collected.

Survey details

Use the following information to complete the survey:

· Distribution date: Tuesday, Feb. 6, 2024.

· Completion deadline: Tuesday, Feb. 20, 2024.

· Anonymity: Responses will remain anonymous.

· How to participate: Choose a member of your team to complete the survey sent via email.

Why your input matters

DHS will use the survey feedback to guide decisions on the LTSS Improvement Tool’s future use, including:

· The frequency of data provision to lead agencies.

· How DHS and lead agencies use the data.

· Necessary revisions to improve data quality.

Key survey sections

In the survey, you will:

1. Evaluate statements about the LTSS Improvement Tool.

2. Rate the usefulness of data for specific purposes.

3. Share feedback about the LTSS Improvement Tool’s effectiveness to identify ways to improve it.

4. Provide insights about disaggregated data points, ways to refine the tool and distribution frequency.

Reminder: Requirements for case managers

Since DHS launched the LTSS Improvement Tool in fall 2017, case managers are required to annually complete the survey to gather feedback from people who receive services. The LTSS Improvement Tool is an important part of the MnCHOICES Support Plan application. For more information, refer to DHS – Frequently asked questions about the LTSS Improvement Tool.
