MnCHOICES legacy systems retirement
On Oct. 1, 2024, DHS will remove legacy assessment and support planning tools used by lead agencies and other assessment/support planning tools used by managed care organizations (MCOs) from DHS – Searchable document library (eDocs). We will discontinue these documents to make sure all agencies use the MnCHOICES revision.
Effective Oct. 1, 2024, lead agencies will no longer be able to use the following documents to complete their work:
Background information
On July 1, 2024, DHS started Phase 4 (the final transition period) of the extended partner-advised rolling launch of the revised MnCHOICES application. Phase 4 ends on Sept. 30, 2024. During Phase 4, lead agencies should:
Effective Oct. 1, 2024, legacy systems will no longer be available.
Exception: Six counties currently contract with public health nurse agencies that use the legacy personal care assistance (PCA)/Community First Services and Supports (CFSS) document to conduct PCA/CFSS reassessments. DHS will work with these counties to move their work to the MnCHOICES revision after Sept. 30, 2024.

Terminology
Legacy systems: MnCHOICES 1.0 assessment and support plan applications, legacy assessment and support planning tools and other assessment/support planning tools used by MCOs.
MnCHOICES revision: The vendor-hosted version of MnCHOICES that will replace all legacy systems.
