To: People who use personal care assistance (PCA) or the Consumer Support Grant (CSG), their families, providers, lead agencies and other interested parties

Purpose: To remind interested parties about Community First Services and Supports (CFSS) implementation details
Effective: Oct. 1, 2024

CFSS implementation begins Oct. 1, 2024

Effective Oct. 1, 2024, people using PCA and CSG will start their transition to CFSS at their next assessment.

Process overview

If the person is eligible at assessment and chooses CFSS services:

1. The lead agency provides the person with:

  • · DHS – List of consultation services providers
  • · CFSS Information Sheet – English, DHS-8477A-ENG (PDF) or translated version:
    Hmong, DHS-877A-HMN (PDF)
    Karen, DHS-877A-KAR (PDF)
    Somali, DHS-877A-SOM (PDF)
    Spanish, DHS-877A-SPA (PDF)
  • 2. The person chooses a consultation services provider.

    3. The lead agency authorizes consultation services and three months of PCA or CSG to avoid a gap in services at the start of the new eligibility span.
    Note: The lead agency must follow guidelines on CFSS Manual – PCA/CFSS unit determination for both the three-month PCA/CSG authorization and the CFSS authorization.

    4. The person writes their service delivery plan with help from the consultation services provider, if desired.

    5. The lead agency approves the service delivery plan and authorizes services.
