Training announcement: Elderly Waiver (EW) Foundations 2: Assessment, Support Planning and Rates

Description

This 45-minute, self-paced online course introduces EW assessment, support planning and rates.

EW Foundations 2 is the second of three courses. EW Foundations 1: Administration and Eligibility is available in TrainLink. We will announce EW Foundations 3 when it is available.

Learning objectives

After the training, participants will be able to:

  • Identify examples of a person-centered approach.
  • Describe the use of MnCHOICES assessment information in support planning.
  • Summarize the support planning process.
  • List the required steps for service authorization.
  • Locate and apply current rates for EW services, including residential services.

Target audience

People who work with home and community-based services for older adults, including:

  • Care coordinators.
  • Case managers.
  • Assessors.
  • Other interested parties.

Registration information

Register using TrainLink

You must have a unique key to register and receive credit for training. If you do not have a unique key, complete the Unique Key Request Form.

Steps

  • Go to the Older Adult Programs Learning Center in TrainLink.
  • Sign on in the upper-right corner.
  • Enter your unique key.
  • Select Course catalog search.
  • Search for “Elderly Waiver Foundations 2” or “AASD-EWF2.”
  • Select the course from the search results.
  • Select Start course.
