Purpose: To inform lead agencies that DHS will discontinue the Long-Term Services and Supports (LTSS) Improvement Tools

LTSS Improvement Tools discontinued

DHS will not implement the LTSS Improvement Tools into the revised MnCHOICES application.

Reasons for the change

DHS decided to discontinue the LTSS Improvement Tools after thorough review. We considered lead agency feedback and shifts in federal requirements and priorities.

The LTSS Improvement Tools discontinuation will:

  • · Lessen the workload for lead agencies.
  • · Allow lead agencies to allocate more time and resources to direct service provision.
  • · Simplify support plan visits and make them more person-centered and efficient.

  • · Remove the LTSS Improvement Tools from MnCHOICES.
  • · Update applicable MnCHOICES trainings.
  • · Redirect our resources toward Access Rule compliance.
  • · Develop new quality assurance measures that meet current federal requirements.
  • · Strengthen relationships with lead agencies.
  • Background information

    DHS designed the LTSS Improvement Tools to gather feedback from people who receive services about their experience with long-term services and supports in Minnesota. We integrated the LTSS Improvement Tools into the MnCHOICES Support Plan 1.0 application in 2017. We built the LTSS Improvement Tools into the revised MnCHOICES application but did not release them.
