CFSS Billing for FMS Fee and Failed Background Study (T2040)
CFSS FMS Fee (T2040) Budget Model
The financial management service (FMS) provider must establish the maximum rate(s) for their services and make their service rates public. Find this information at FMS provider information. Use the following HCPCS procedure code and modifier to bill for a Community First Services and Supports (CFSS) FMS fee.

CFSS Failed Background Study (T2040) Budget Model
FMS providers will use the same FMS fee procedure code to bill for a failed background study for the worker the member chooses to provide CFSS services.
First, the FMS provider will submit a background study request for a CFSS worker to the Minnesota Department of Human Services (DHS) on behalf of a member using the budget model. If the background study has failed, then the FMS provider must add the fee for the failed background study to the service authorization by submitting the online form.
Add the Failed Background Study to the Service Authorization
The FMS provider must submit the online form CFSS Technical Change Request (DHS-6893K) to add a new line to the service authorization for a failed background study. The FMS provider will receive an updated service authorization in their MN-ITS mailbox. The FMS provider can then bill Minnesota Health Care Programs (MHCP) for the failed background study fee.
The FMS provider cannot include the cost of fingerprinting and photos when billing for the background study fee.

How to Determine the Results of a Background Study
If the CFSS worker’s background study fails, the Applicant Portal will post a disqualification notice for a failed background study. Individual CFSS workers taking the background study also receive a notice by U.S. Postal Service mail. FMS providers receive electronic notices of background study results via the NETStudy 2.0. Billing Background Studies that Pass
Only FMS providers can bill for background studies that pass. FMS providers can refer to the Agency and Budget Model Billing for PCA/CFSS Personal Care Services (T1019) for instructions on billing for background studies that pass.
Background Studies Using the Agency Model
The CFSS provider agency cannot bill for background studies under the CFSS agency service model. For the agency service model, the background study fee is part of the CFSS provider agency overhead cost.
