Assisted Living Report Card state licensing and investigations results feature available

Effective immediately, users can access the state licensing and investigations results feature in Minnesota’s Assisted Living Report Card.

DHS notified the public of a technical issue with the state licensing and investigations results feature in the July 18, 2025, eList announcement. Recently, Minnesota IT Services (MNIT) resolved the issue.
