Purpose: To remind counties/tribal nations that all people receiving personal care assistance (PCA) on AC must transition to Community First Services and Supports (CFSS) by March 31, 2026
Effective: March 31, 2026

People receiving PCA on AC must transition to CFSS by March 31, 2026

By March 31, 2026, all people receiving PCA on AC must transition to CFSS.

County/tribal nation actions

Before March 31, 2026, counties/tribal nations must make the following updates in MnCHOICES and the Medicaid Management Information System (MMIS):

  • · End PCA services on March 31, 2026, in MnCHOICES support plans for people on AC.
  • · End all T1019 lines that do not have a CFSS model modifier (i.e., U9 or UB) or a CFSS temporary start modifier (i.e., U8) on March 31, 2026, for AC service agreements in MMIS for people on AC.
  • Note: Counties/tribal nations do not need to send people a notice of action because this change is due to legislation. For more information, refer to CBSM – Notice of action.

    Additional county/tribal nation actions for people who will not have an approved CFSS service delivery plan by March 31, 2026

    If a person will not have an approved CFSS service delivery plan by March 31, 2026, the county/tribal nation must complete the following actions before March 31, 2026:

  • · Follow the MnCHOICES and MMIS instructions described above.
  • · Work with the person to find other AC services or informal supports to meet their personal care needs until they have an approved CFSS service delivery plan.
  • · Update the person’s MnCHOICES support plan to add identified services and informal supports, as needed.
  • · Update the person’s service agreement to add identified services, as needed.
