Manage Portfolio - Existing MHCP Providers

Overview
This portfolio is where you will find information on any requests for an organization or individual provider. You cannot make changes to an MPSE portfolio without making a request. Find guides for completing requests in the MPSE Requests section of the MPSE User Manual.
Use this page to manage your requests within your portfolio. Click Create a New Request to make a new request or click on one of the available options located in the Request Actions column to review or make changes to an existing request.
Master Profile
Under Master Profile, the information shown is the most recent profile information that has been approved by the Minnesota Department of Human Services (DHS).
Legal Name
The Legal Name box displays your legal business name if you are enrolled as an organization provider. If you are enrolled as an individual provider, your legal name displays in the format of first name, middle name, and last name.
FEIN
The FEIN box displays your Federal Employer Identification Number (FEIN) if you entered a FEIN in your profile. Your FEIN will be masked for data privacy and will display as **-*******.
SSN
The SSN box displays your Social Security number (SSN) if you entered an SSN in your profile. Your SSN will be masked for data privacy and will display as ***-**-****.
Ownership Type
The Ownership Type box only displays if you are an organization provider. This box will display the ownership type you selected when entering your organization information. Go to the Manage Organization Information page for information on ownership type.
Last Profile Update
Last Profile Update shows the date that your profile was last updated.
Profile Actions
The Profile Actions shown are the options available to users and appear based on security access assigned by the designated MN–ITS administrator. The actions available allow users only to view their profile and does not allow options to make changes to a profile.
View Profile
Click View Profile to view a profile in a read-only state by moving from one screen to the next. Users are allowed to view the content for each page, but unable to make changes.
Summary Report
Click Summary Report to view and print a summary report of a profile. The summary report contains detailed information that reflects your current Minnesota Health Care Programs (MHCP) record and provides your entire profile in the form of a document that is printable and may be used to file for future reference.
Returned Requests
Under the Returned Requests section, you will see a link to go to the Returned Requests page. The Returned Requests page displays a list of all requests that DHS has returned for more information. This section will only appear if you have had requests returned to you.
Manage Organization to Individual Affiliations
Under the Manage Organization to Individual Affiliations section, you will see a link to go to the Manage Existing Organization to Individual affiliations page. From the Manage Existing Organization to Individual affiliations page you will be able to review and manage your existing affiliations and add new affiliations.
Revalidations
Under the Revalidations section, you will see a link to go to the Manage Enrollment Records Due For Revalidations page. From the Manage Enrollment Records Due For Revalidation page you will be able to review a list of enrollment records with a due date for revalidation assigned to it.
Requests
The Requests section shows all of your requests that exist within your portfolio. Each request will be in their own individual row and will display information or options related to the request including the Submit Date, Status/Outcome, Request Information, Request Details, and Request Actions.
Note that requests made on behalf of individual providers affiliated to your organization will be in a different section of the Manage Portfolio page.
Submit Date
Under the Submit Date column, the date you submitted your request will appear in MM/DD/YYYY format. A date will only appear in this column if the request was submitted to MHCP.
Status/Outcome
Under the Status/Outcome column, the status of the request will appear. A list of potential statuses can be found on the status page of the MPSE User Manual.
Request Information
Under the Request Information column, three pieces of information appear including the type, indicators requestor, and request ID.
The type displayed is the type of request selected on the Select Request Type page. A list of request types can be found on the MPSE Requests page of the MPSE User Manual.
The indicators requestor is the user ID of the MN–ITS account that is currently assigned to the request and is the only person that can edit that request when it is in draft status or delete the request. To change the user ID in the indicators requestor section to the current user ID viewing the MPSE portal, click the Assign to Self link in the Request Actions column of the request.
The request ID is used to identify your request to MHCP and you may be asked for this number when talking to DHS staff about your request.
Request Details
Under the Request Details column, a summary of the changes made as part of the request will be displayed.
Request Actions
Under the Request Actions column, the list of actions that you can take on a request will display. The available actions that appear are determined by your user role and the status of the request.
View Request
Click View Request to review the request listed in that row. You will be able to view the content for each page within the request, but unable to make changes.
Summary Report
Click Summary Report to view and print a Summary Report of a profile. The Summary Report contains detailed information that reflects your current MHCP record and provides your entire profile in the form of a document that is printable and you can file for future reference.
View Differences Report
Click View Differences Report to review a report of how your requested changes will change your current profile if they are completed. The Differences Report will show you a side-by-side comparison of your current profile and your future profile which will include your requested changes.
Edit
Click Edit to make changes to your request.
Delete
Click Delete to delete your request.
Assign to Self
Click Assign to Self to assign the request to yourself to make changes or to complete the request.
Revert to Draft
Click Revert to Draft to return the request to draft mode. This allows you to edit your request. You can only revert a request to draft mode if the request is in the Pending Review status.
You have the option to revert your request to a draft after you have submitted your request for approval but before DHS has started their review of your request. Reverting your request to a draft enables the Edit option on your request. However, once DHS begins the review of your request, you will no longer be able to revert that request to a draft to make changes.
Create a New Request
Click Create a New Request to go to the Select Request Type page to make a change to your profile, enrollment records and affiliations.
Affiliation Requests for Individual Providers
The Affiliation Requests for Individual Providers section shows all of your requests made on behalf of individual providers affiliated to your organization. Each request will be in their own individual row and will display information or options related to the request including the Submit Date, Status/Outcome, Request Information, Request Details, and Request Actions.
Submit Date
Under the Submit Date column, the date you submitted your request will appear in MM/DD/YYYY format. A date will only appear in this column if the request was submitted to MHCP.
Status/Outcome
Under the Status/Outcome column, the status of the request will appear. A list of potential statuses can be found on the status page of the MPSE User Manual.
Request Information

Request Actions
Under the Request Actions column, there will be a link to the Manage Individual Portfolio page. Click the Manage Individual Portfolio link to go to the individual provider’s portfolio that the request was made under.
Manage Completed Requests
Under the Manage Completed Requests section, there will be a link labeled Completed Requests. Click the Completed Requests link to go to the Manage Completed Requests page.
