Manage Site Visits

Overview

Use this page to view or edit site visits for this enrollment record. You can only edit the information on this page if you are in an MPSE Request.

Providers whose enrollment record type is considered moderate or high risk for fraud, waste or abuse are required to complete a site visit at the time of their initial enrollment, re-enrollment and at revalidation of their enrollment. If you have more than one location, each location must complete a site visit.

Some limited risk provider types may be considered a moderate or high risk if they are providing certain services that are considered a higher risk for fraud, waste or abuse. If you are providing these services, Minnesota Health Care Programs (MHCP) will consider you a moderate or high risk and will require a site visit to be completed.

To review the assigned risk levels for each provider type, services that raise your risk level, risk level requirements and additional information on site visits, review the MHCP Provider Manual.

If you have had a site visit completed by Medicare within the last five years or another state Medicaid agency within the last 12 months, you do not need to complete another site visit, but you must provide documentation of your completed site visit.

Enrollment Record Information

This is a recurring section that appears on multiple MPSE pages for informational purposes only. Refer to Enrollment Record Information on the Recurring Items page of the MPSE User Manual.

Manage Site Visits

The Manage Site Visits section displays information related to your site visits. If no site visits have been uploaded for this enrollment record, this section will not have any information to display.

View
Click View to go to the Site Visit Information page. From this page, you can view your site visit information. You cannot edit information on this page while in view mode.

View/Edit
Click View/Edit to go to the Site Visit Information page. From this page, you can view and edit your site visit information.

Delete
Click Delete to delete your site visit information.

Add a Site Visit

Click Add a Site Visit to go to the Site Visit Information page.

Continue

Click Continue to go to the next page.

Site Visit Information

Overview

In this section of MPSE, you can add site visit information to your enrollment record.

Portfolio/Profile Information

This is a recurring section that appears on multiple MPSE pages for informational purposes only. Refer to Portfolio/Profile Information on the Recurring Items page of the MPSE User Manual.

Enrollment Record Information

This is a recurring section that appears on multiple MPSE pages for informational purposes only. Refer to Enrollment Record Information on the Recurring Items page of the MPSE User Manual.

Site Visit Information

In the Site Visit Information section, provide information about your site visit.

Site Visit Type

This field is for office use only. MHCP will complete the site visit type when reviewing your request. Your site visit type will be one of the following:

  • · Other
  • · Pre-enrollment
  • · Re-enrollment
  • · Revalidation
  • Site Visit Other Description

    This field is for office use only. MHCP will complete the Site Visit Other Description if your selected site visit type is “Other” and will identify why the site visit was completed.

    Site Visit Status

    This field is for office use only. MHCP will complete your site visit status when reviewing your request to add a site visit to this enrollment record.

    Site visit status appears as “Pending” by default until MHCP reviews and updates your request. Your site visit status will be one of the following:

  • · Approved
  • · Denied
  • · Other
  • · Pending
  • Referred to/Performed By

    Select Referred to/Performed by to identify who has completed or who will complete your site visit using the drop-down list. The options available are:

  • · Department of Human Services (DHS)
  • · Medicare
  • · State list
  • If DHS completed your site visit, select DHS. If Medicare completed your site visit, select Medicare. If your site visit was completed or will be completed by another state Medicaid program, select the appropriate state.

    Referral Date

    This field contains the date the enrollment record was referred for the site visit and is for office use only. MHCP will add the referral date when your site visit information has been reviewed.

    Completion Date

    If your site visit has been completed, enter the date your site visit was completed in the format of MM/DD/YYYY in the Completion Date field.

    Site Visit Documentation

    The Site Visit Documentation field shows the documentation uploaded to support the completed site visit. Click the page icon to open the document in a new tab.

    Remove Upload

    Check the Remove Upload box to remove the document that is currently uploaded. This will happen automatically if there is an existing document and you upload a new document. This box will not appear if there is no document uploaded in the Site Visit Documentation field.

    Upload Site Visit Documentation

    Use the Upload Site Visit Documentation field to upload documentation you have to support your request to change your profile identifier. Supporting documentation is not required. Click on the words Upload Site Visit Documentation or click on the icon shaped like an eye to open a dialogue box on your computer which will allow you to select a document to upload. MPSE only allows the following document types for upload: PDF, JPG, and PNG.

    Continue

    Click Continue to save your changes and go to the next page.

    Cancel

    Click Cancel to go back to the previous page.
