Direct Support Worker Affiliations

Overview
Affiliations refers to the professional relationship between an organization and an individual provider. All direct support workers (DSWs) must be affiliated to at least one organization. You can only make changes to this page when you are in a Direct Support Worker Affiliation Request.
A direct support worker is any one of the following:
Credentials and requirements vary depending upon the service(s) added to the Services page of the DSW’s enrollment record. Refer to the MHCP Provider Manual for enrollment requirements.
Use this page to view existing affiliations, create a new affiliation, or make changes to an existing affiliation.
Manage Organization to Direct Support Worker Affiliations
Portfolio/Profile Information
This is a recurring section that appears on multiple MPSE pages for informational purposes only. Refer to Portfolio/Profile Information on the Recurring Items page of the Minnesota Provider Screening and Enrollment (MPSE) User Manual.
Enrollment Record Information
This is a recurring section that appears on multiple MPSE pages for informational purposes only. Refer to Enrollment Record Information on the Recurring Items page of the MPSE User Manual.
Modified Organization to Direct Support Worker Affiliation
The Modified Organization to Direct Support Worker Affiliation section displays information on your current affiliation request. If you do not have a current request, this section will not appear.
View/Edit
Click View/Edit to go to the Manage Organization to Direct Support Worker Affiliation page. From this page, you can view and edit your affiliation request.
Delete
Click Delete to delete your current organization to direct support worker affiliation.
All Direct Support Worker Affiliations
The All Direct Support Worker Affiliations section displays information related to any existing organization to direct support worker affiliations for the DSW’s profile you are managing. If there are no organization to direct support worker affiliations for this DSW, this section will not have any information to display.
View/Edit
Click View/Edit to go to the Manage Organization to Individual Support Worker Affiliation page. From this page, you can view and edit your affiliation information.
View
Click View to go to the Manage Organization to Direct Support Worker Affiliation page. From this page, you can view your affiliation information. You will not be able to make any changes while in View mode.
Create New Affiliation
Click Create New Affiliation to go to the Manage Organization to Direct Support Worker Affiliation page. From this page, you can create a new affiliation.
Continue
Click Continue to save your changes and go to the next page.
Manage Organization to Direct Support Worker Affiliation
Portfolio/Profile Information

Enrollment Record Information
This is a recurring section that appears on multiple MPSE pages for informational purposes only. Refer to Enrollment Record Information on the Recurring Items page of the MPSE User Manual.
Affiliation Information
in the Affiliation Information section, give information for your organization to direct support worker affiliation request.
Affiliation Request Type
Select your affiliation request type from the Affiliation Request Type drop-down list. The affiliation request types that are available for you to select from are:
Employee Hire Date
Enter the date your organization hired this DSW in the format of MM/DD/YYYY in the Employee Hire Date box.
DSW Organization Enrollment Record
Select the enrollment record you are requesting to affiliate this DSW to from the DSW Organization Enrollment Record drop-down list.
Start Date
Enter your affiliation start date in the format of MM/DD/YYYY in the Start Date box.
End Date
Enter your affiliation end date in the format of MM/DD/YYYY in the End Date box. Only add an end date if this DSW no longer works for your organization.
Continue
Click Continue to save your changes and go to the next page.
Cancel
Click Cancel to go back to the previous page.
