Affidavit of Mailing and Notice (DHS-8348A)
Follow the instructions for (DHS-8348A) (PDF) to show the court that you have given proper notice to initiate probate.
1. Fill in the header.· Select your county name from the top left dropdown.· Enter the decedent’s (deceased person’s) name on the left.· Enter the court district number in the top right. If you are unsure of your district, look up the court district. · Since you are opening a new court file, you do not need to enter a file number.· Check the box of the document you are sending under Affidavit of Mailing. If initiating probate, then check the “Petition” box.2. Fill in the body.· Enter your county’s name (or the county where you are signing, if not signing in your county).· Place your name (or the name of the person who sent the notices) in the first blank.· Enter the date the notices were sent in the second blank and select the county the notices are being sent from.· Enter what the notices were sent for in the fourth blank. For initiating probate, it will be the petition.· Enter the city the notices were sent from in the fifth blank.3. Fill in the information on the recipients of the notice.· Put each interested party’s name and address in the table. Add or delete rows as needed. 4. Put your office contact information in the lower right box.5. Date and sign the document.
