16.44 Accuracy Review Reporting

The Department of Children, Youth, and Families (DCYF) identifies specific policies for review in each quarterly review period. Each policy is reviewed for a minimum of two reporting quarters and continues until an acceptable level of error is achieved. DCYF randomly selects cases to be reviewed in each quarterly review period. Agencies must report accuracy review information for selected cases to DCYF.
Notification to CCAP agencies
DCYF will inform CCAP agencies when a case is selected for the reporting quarter. Information about cases selected and policy review required is sent to agencies approximately once a quarter. The cases selected are posted on SIR. The number of cases to be reviewed by each agency is based on the caseload size of the agency.
Agency reporting
Accuracy reviews are reported to DCYF using forms posted on eDocs (7356, 7356A, or 7356B). The email notification for each quarter of Accuracy Reporting will contain the link to the eDoc used for that quarter. A new form will be created for each policy reviewed. CCAP agencies must complete a form for each case selected in each reporting quarter. The person completing the form must not be the same person that determined eligibility or processed the most current redetermination on the case.
Submission timeline
Accuracy review forms must be submitted for all cases selected to DCYF within thirty days of DCYF notification.
Internal review for best practice
Agencies are encouraged to conduct their own reviews of policies used when establishing eligibility, authorizing care, or making payments to improve case accuracy and address common worker errors.
