Lead Agency Review website

A description of the DHS Home and Community-Based Services (HCBS) Lead Agency Review process. It includes information specific to person-centered practices, such as the:

Person-Centered, Informed Choice and Transition Protocol, DHS-3825 (PDF)

Specific, person-centered guidelines support planners must use in planning. Adherence to the protocol is monitored by the Lead Agency Review process.

Regional Resource Specialists (RRS)

Disability Services Division staff, located across the state, who advise and assist on a variety of matters, including person-centered practices. RRSs help convene regional communities of practice for support planners and others interested in developing person-centered practice skills.

Support planning professionals learning community

DHS-led resource for support planners, assessors and others. It hosts monthly webinars to share information and resources about person-centered practices. PowerPoints from the webinars are stored in the Disability Services Division training archive (look for “support planning professionals learning community”).

Community-Based Services Manual (CBSM)

The CBSM contains guidance and explanation of DHS home and community-based services program policies.

Support Development Associates’ Go-To Guide for Person-Centered Thinking Skills

Support Develop Associates, LLC, is a resource for learning the fundamentals of person-centered practices. The site shares a number of tools people can use as person-centered practice aids.

Person-Centered Organizational Development Tool (PDF)

An assessment tool from the University of Minnesota’s Institute on Community Inclusion. It can help you assess your organization’s strengths and areas of need regarding person-centered practices.

Disability Hub MN

Disability Hub MN is a free statewide resource network that helps people with disabilities solve problems, navigate the system and plan for their future. It has options and tools for managing health, benefits, work, housing and more. It includes information on:

  • Disability Benefits 101, or DB101, that helps people explore ways to balance benefits and work. People use the site to plan ahead and explore how work and benefits go together.
  • Housing Benefits 101 that helps people explore housing options and learn about programs that can make housing more affordable. People use this site to discover what works and make a plan for getting there.

Positive Supports Minnesota

Information about positive supports, including person-centered practices. Positive supports are approaches that offer respectful, supportive and effective ways to help people make positive changes in their lives.
