Post Permanency Reassessments – Minnesota Assessment of Parenting for Children and Youth (MAPCY)

  • Reassessments may not be requested or conducted:
  • Between the time the Adoption/Kinship benefit agreement becomes fully executed and the adoption/TPLPC is finalized
  • If less than 6 months has passed since the previous assessment
  • Exception: At-risk reassessment when disability manifests
  • Family or county contacts DHS Adoption/Kinship worker to request a reassessment
  • Must be 6 months since the last assessment (unless it is an at-risk conversion and disability has manifested)
  • Must be in writing from the family
  • DHS locates the child in SSIS-DHS and initiates a reassessment to the agency of financial responsibility
  • The child must be cleared to SWNDX before the reassessment can be initiated
  • If the child already exists in SWNDX (for the post-adoptive identity), DHS worker will clear the post adoptive identity to that existing SWNDX #
  • If the post adoptive identity is not found in SWNDX, the child will be cleared in SSIS-DHS with a new SWNDX #
  • Financially Responsible agency manages the log for new requests
  • Log in SSIS-Worker that allows agency to view pending reassessments (Searches/Logs – Post Permanency Reassessment Log)
  • Request to agency will include both the pre-adoptive/kinship and post-adoptive/kinship names and SWNDX numbers and current MAPCY level
  • Once DHS has initiated the Post Permanency Reassessment, the child with the post adoptive SWNDX # (as cleared by DHS) will display on the Post Permanency Log in the local agency
  • If the post adoptive client exists in the local agency AND has the same SWNDX # as the Post Permanency Reassessment Log, there will be a plus sign (+) next to the child’s name on Post Permanency Reassessment Log. Allowing the local agency to expand the person tree for that child
  • If the post adoptive client exists in the local agency AND does not have a SWNDX # OR has a different SWNDX # than the one the Post Permanency Reassessment Log, there NOT will be a plus sign next to the child’s name on Post Permanency Reassessment Log
  • If no SWNDX # - The post adoptive client should be cleared to the SWNDX # on the Post Permanency Reassessment Log
  • If SWNDX # but different than the one on the Post Permanency Reassessment Log – data fix is needed
  • MAPCY should not be completed until the client in SSIS Worker and the Post Permanency Log match
  • If a MAPCY is completed with non-matching SWNDX #s, the system will not recognize that they are the same child and will NOT pick up the reassessed level.
  • If SWNDX #s do not match, send both SWNDX # to the SSIS Helpdesk for a data fix before creating the MAPCY
  • Based on analysis – the data integrity team will determine the KEEPER SWNDX # and make the appropriate changes to Worker, SWNDX and SSIS DHS
  • SSIS-Worker allows the completion of a new MAPCY and compares the results of the original MAPCY
  • In the MAPCY Setup Tab, the purpose is “Post Permanency Reassessment (NAA or NKA)”
  • Approve MAPCY (if the child is eligible for an Extraordinary Levels (EL)increase, the EL must be approved by DHS)
  • If the results are the same or lower, notifications within SSIS-Worker are available to notify adoptive parent/relative custodian
  • Status within the Post Permanency Reassessment Log changes to “Reassessment Not Implemented”
  • If a higher benefit level is determined and approved
  • SSIS-Worker automatically sends a notification with the proposed benefit level increase to SSIS-DHS
  • Status within the Post Permanency Reassessment Log changes to “MAPCY Completed”
  • Financially responsible agency must send a copy of the MAPCY Rating Report to the DHS Adoption Assistance worker.
  • DHS reviews the MAPCY Rating Report
  • DHS creates and sends out an amendment to the adoption/kinship benefit agreement to the family for signature (for an at-risk conversion, amendment also sent to agency for signature, who then forwards to family)
  • Once signed and returned, DHS signs the amendment, sets up new payment and issues back payment if necessary (Status on the log changes to “Amendment Completed”)
  • New rates goes into effect the date DHS Adoption Assistance signs the amendment.
