Family Assessment and
Child Protection Service Plans
The Family Assessment Service Plan (FA Plan) and the Child Protection Service Plan (CP Plan) are created within the first 30 days of service plan provision, and are to be reviewed with the family every 90 days. Both service plans merge SDM Strengths and Needs Assessments, and the CP Plan additionally merges Risk Assessments. Therefore, it is advisable to complete these documents first from the Decision Tools folder.
To Open and Access a New Family Assessment or Child Protection Service Plan:
- Right-click on the Service Plans folder.
- Select New State Service Plan.
- The Setup tab displays.
- Click on the Document template drop-down menu and select either Family Assessment Service Plan or Child Protection Service Plan as is applicable.
To Complete the Setup Tab of a New Family Assessment or Child Protection Service Plan:
- Click anywhere on the Setup tab screen to enable data entry.
- Complete the fields on the Setup tab.
- Click Save.
- Click on the Document tab.
The Navigation bar displays to the left of the service plan screens. It operates very similarly to the Tree View. The Document tab opens to the Title Page. The screen that displays to the right of the service plan is indicated by the blue arrow in the Navigation bar. Move to the next screen by clicking in the Navigation bar, or accessing the double-arrows at the bottom right of the screen.
A red checkmark displaying to the left of a node indicates that the Complete check box was selected. The Complete box is located at the bottom of every Document tab screen, and must be selected before that the screen prints on the plan.
To Complete the Document Tab of a New Family Assessment or Child Protection Service Plan:
- Click in all check boxes that reflect answers to questions regarding the child,
the family, and the resource family. - Click on the Editor button, or type text directly into the Editor box, to answer the questions on the OHPP screens.
- Make applicable selections in drop-down menus on the various OHPP screens.
- When the fields on a screen you wish to print are complete, click the Complete check box at the bottom of the screen.
- Type the names of required signers in the Signature Setup screen.
- Review the Navigation bar to ensure that each screen you wish to print displays with a red checkmark next to the corresponding node.
- Click Save.
Hint: In order for text to print on the final service plan, the Complete box at the bottom of each screen must be checked. Once the Complete box is checked, a red checkmark displays in the Navigation bar.
Hint: Right-click on the Document tab, or click on the FA or CP Plan in the Tree View, to print the plan. Activate the service plan once signatures are obtained.
