About the MnCHOICES interagency contact point
The MnCHOICES interagency contact point (MICP) list provides a dedicated, secure phone number, email and fax number for each lead agency using MnCHOICES to complete assessments for Minnesota residents. The MICP makes interagency communications easier. It does not replace existing agency support mechanisms for users (e.g., mentors) or intake/initial contact processes.

How do lead agencies submit MICP information and updates?
Lead agencies complete the online Update MnCHOICES interagency contact point list form to update their lead agency's MICP information. Do not use this online form to share information about specific cases or include nonpublic or protected data in your submission.
