Recent CBSM updates
In June 2024, DHS made updates to the following pages in the CBSM.

ICS settings | We made the following changes on CBSM – Resource: Integrated community supports (ICS) settings: |
Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older | We updated the list of providers on CBSM – Resource: Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older to reflect provider enrollment status as of June 1, 2024. |
Lead agency employment liaison contact list | We updated information on CBSM – Lead agency employment liaison contact list for the following counties: |
Temporary waiver exits and restarts: MMIS actions | We updated CBSM – Temporary waiver exits and restarts: MMIS actions and made the following changes: For more information, refer to the June 11, 2024, eList announcement. |
Corporate foster care moratorium liaison contact list | We updated information on CBSM – Corporate foster care moratorium liaison contact list for Douglas County. |
ICLS | We clarified information on CBSM – Individual community living supports (ICLS) about when a home/apartment owner can provide ICLS to a person living in their home. This update was a clarification and did not change ICLS policy. |
MnCHOICES | We comprehensively reviewed CBSM – MnCHOICES and added content about the revised MnCHOICES application. |
NF LOC criteria guide | We posted CBSM – Nursing facility level of care (NF LOC) criteria guide, which replaces a PDF version of the guidance (DHS-7028). |
Training
AASD
AASD and the Minnesota Board on Aging offer several online courses for lead agency staff and for those looking to learn more about how to better serve and support older adults more broadly. All courses are available on the Older Adult Programs Learning Center, located on TrainLink under “Older Adult Programs.”

DSD
Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.
The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, visit TrainLink and search for course code “Housing_CM”.
