How to use MnCHOICES for people enrolled in managed care and on a disability waiver

DHS updated policies to create a new workflow for the MnCHOICES Assessment and Support Plan for people enrolled in managed care and on a disability waiver. For more information, refer to:

· New workflow guidance as shared in the Aug. 23, 2024, release updates DHS emailed to MnCHOICES mentors.

· New billing guidance in the “billing when in a dual role” section on CBSM – Care coordination.

We also are developing a smart guide called “Guidance for assessment and support plan for dual roles.” It will have detailed instructions for how to use MnCHOICES, including how to bill for time.
