In December 2024, DHS made updates to the following pages in the CBSM.

Information about update

ICS settings

We updated the list of settings on CBSM – Resource: Integrated community supports (ICS) settings.

Case management aide

We comprehensively reviewed CBSM – Case management aide and made the following changes:

  • · Expanded definitions to include Alternative Care (AC) and Essential Community Supports (ECS).
  • · Added more examples of covered services.
  • · Added a new section for existing information about authorization, rates and billing (no policy changes).
  • Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older

    We updated the list of providers on CBSM – Resource: Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older to reflect provider enrollment status as of Dec. 1, 2024.

    Lead agency employment liaison contact list

    We updated information on CBSM – Lead agency employment liaison contact list for the following counties:

  • · Chisago.
  • · Isanti.
  • · Scott.
  • Requirements for a person’s own home

    We updated CBSM – Requirements for a person’s own home and made the following changes:

  • · Updated a legal authority reference.
  • · Added definitions for community-living setting, direct financial interest, indirect financial interest, controlling individual and license holder.
  • · Clarified information about general requirements for a person’s own home.
  • Training

    AASD

    AASD and the Minnesota Board on Aging offer online courses and training materials for staff from counties, tribal nations, managed care organizations, providers and other interested parties about how to better serve and support older adults.

    All courses are available in the Older Adult Programs Learning Center in TrainLink.

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, visit TrainLink and search for course code “Housing_CM”.
