CFSS implementation updates

DHS began the transition from PCA and CSG to CFSS on Oct. 1, 2024. We will provide regular updates throughout the transition.

Policy manual

DHS made the following changes:

  • · Updated CFSS Manual Overview of the PCA and CFSS process for people not receiving services through a managed care organization (MCO) with more detailed information.
  • · Posted CFSS Manual – Overview of the PCA and CFSS processes for people receiving services through an MCO.
  • · Updated CFSS Manual – CFSS consultation services overview to clarify covered and billable tasks and expand the list of examples.
  • Forms and documents

    DHS posted CFSS Individual Service Delivery Plan (short version), DHS-6893Q (PDF), an abbreviated version of CFSS Individual Service Delivery Plan, DHS-6893P that people and providers must use to make a service delivery plan for a 45-day temporary start of CFSS services.
    Note: People and providers optionally can use DHS-6893Q instead of DHS-6893P as the plan available in their home or service location for their workers to reference. People must use DHS-6893P to submit their service delivery plan to the county/tribal nation for approval.

    DHS made the following changes to CFSS Individual Service Delivery Plan, DHS-6893P:

  • · Updated calculations to reflect the 2026 cost-of-living adjustments (COLA) and enhanced rate increases.
  • · Added a revisions section to describe changes since the person’s last version.
  • · Updated the worker budget section to comply with Minnesota Paid Leave requirements.
  • · Added a section to indicate interest in shared care.

    Resources

    DHS updated DHS – CFSS and PCA frequently asked questions and posted DHS – CFSS FAQ for people who use services.

    MMIS instructions

    For details about all manual updates, refer to DSD MMIS Reference Guide.

    MMIS system updates

    DHS continues to test and fix bugs in MMIS. Counties/tribal nations can use Service Agreement and Screening Document (SASD) Support Team Portal, DHS 3754 to report issues to DHS.

    Reminders

    DHS reminds counties/tribal nations that all people receiving PCA on the Alternative Care (AC) program must complete the transition to CFSS by March 31, 2026.
