Purpose: To announce updates DHS made to the Community-Based Services Manual (CBSM) in May 2026

For Alternative Care (AC), Essential Community Supports (ECS) and Elderly Waiver (EW), email DHS.AASD.HCBS@state.mn.us.     We comprehensively reviewed CBSM – AC fees and made the following changes:

  • · Clarified content throughout the page.

    Lead agency employment liaison contact list

    We updated information on CBSM – Lead agency employment liaison contact list for the following counties:

  • · Beltrami.
  • · Lac qui Parle.
  • · McLeod.
  • Resource: Life-sharing matching process and ongoing support options

    We updated the authorization section of CBSM – Resource: Life-sharing matching process and ongoing support options to add a link to RMS User Manual – Family residential services (FRS): Disability Waiver Rate System (DWRS) tiered-rate service.

    Shared service option for HCN

    We comprehensively reviewed CBSM – Shared service option for home care nursing (HCN) and made the following changes:

  • · Added a definition of HCN to align with content on CBSM – HCN.
  • · Clarified information about covered services, non-covered services and provider responsibilities.
  • · Moved information about covered locations to CBSM – HCN because it applies to all HCN services.
  • Independent LPN and RN in HCN

    We comprehensively reviewed CBSM – Independent licensed practical nurse (LPN) and registered nurse (RN) in HCN and made the following changes:

  • · Merged information about LPNs and RNs into one page.
  • · Added definitions of LPN and RN.
  • · Clarified content throughout the page.
  • Extended home care services

    We comprehensively reviewed CBSM – Extended home care services and made the following changes:

  • · Updated the organization of the page to better explain the information for each extended service.
  • · Removed information about personal care assistance (PCA) and Community First Services and Supports (CFSS) because that information lives on CFSS Manual – Extended PCA/CFSS services.
  • · Clarified information throughout the page.
  • Positive support services staff levels

    We updated CBSM – Positive support services staff levels and made the following changes:

  • · Removed old terminology for “behavioral support professional,” “behavioral support analyst” and “behavioral support specialist.”
  • · Added a new licensing requirement option for waiver positive support professionals and waiver positive support analysts.
  • · Added a timeline (i.e., 90 calendar days of hire) for waiver positive support analysts and waiver positive support specialists to meet certain requirements.
  • · Added an option for people who meet the qualifications for a positive support professional to substitute those qualifications for the waiver positive support specialist experience and requirements.
  • Transition services provider contact list

    We updated information on CBSM – Transition services provider contact list for the following providers:

  • · Access Health, LLC.
  • · Allied Community Services.
  • · Peoples Development Services.
  • · People Project.
  • · Promise Comprehensive Services.
  • · Thomas Allen, Inc.
  • We added the following new providers:

  • · Secure Housing Solution Services.
  • · Solace Healing, LLC.
  • We removed Metro Care Human Services.

    Assisted living facilities licensed by MDH and enrolled to deliver customized living by DHS

    We updated CBSM – Assisted living facilities licensed by MDH and enrolled to deliver customized living by DHS and made the following changes:

  • · Removed the column about bed capacity.
  • · Added notes to the size and customized living (CL) moratorium columns to clarify that lead agencies must use the MDH Health Care Provider Lookup to look up bed capacity and determine whether the limitation applies.
  • · Updated the list of providers.
  • CSG transition to CFSS

    We updated CBSM – Consumer Support Grant (CSG) transition to CFSS to reflect the CFSS transition extension announced in the May 12, 2026, eList announcement.

    HCN

    We updated CBSM – HCN and made the following changes:

  • · Added a section to explain applicability for CDCS.
  • · Added a section about covered service locations.
  • Home care, PCA and CFSS overview

    We comprehensively reviewed CBSM – Home care, PCA and CFSS and made the following changes:

  • · Clarified information in the background section.
  • · Updated links for CFSS.
  • Training

    AASD

    AASD and the Minnesota Board on Aging offer online courses and training materials for staff from counties, tribal nations, managed care organizations, providers and other interested parties about how to better serve and support older adults.

    All courses are available in the Older Adult Programs Learning Center in TrainLink.

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, go to TrainLink and search for course code “Housing_CM”.
