New web-based MMIS LTSS Manual

DHS is excited to announce the new web-based Medicaid Management and Information System (MMIS) Long-Term Services and Supports (LTSS) Manual. The manual is a resource for lead agency assessors, case managers and care coordinators who work with long-term care (LTC) screening documents and service agreements in MMIS.

Since 2023, DHS worked to:

  • · Consolidate three existing PDF manuals into one web-based tool.
  • · Reorganize content to make it easier for users to find the information they need.
  • · Move policy content to appropriate policy manual pages.
  • As a result, DHS will discontinue the following eDocs:

  • · Instructions for Completing and Entering the LTCC Screening Document and Service Agreement into MMIS, DHS-4625.
  • · Instructions for Completing and Entering the LTCC Screening Document and Health Risk Assessment into MMIS for the MSC+ and MSHO Programs, DHS-4669.
  • · Instructions for Completing and Entering the Health Risk Assessment into MMIS for the Special Needs Basic Care (SNBC) Program, DHS-5020A.
  • Updates to manual content

    While DHS worked on this project, we put updates to manual content on hold. You might notice some information that needs updates. Now that we have launched the new manual, we will focus on updating outdated content. We will announce updates via MMIS LTSS Manual – Updates to the manual and eList announcements.
