FAMILY SUPPORT GRANT PROGRAM

The Family Support Grant Program is a state-funded program that provides cash grants to families with children with disabilities who live, or will live, in their family home. The home is considered to be the least restrictive and most normalized placement for children with disabilities. However, many families who have children with disabilities living at home incur higher-than-average expenses that are directly related to the disability. The grants provide families with the flexibility to purchase an array of supports and services to meet specific needs.
The goal of the Family Support Grant Program is to prevent or delay the out-of-home placement of children with disabilities and to promote family health and social well-being by enabling access to family-centered services and supports.
Families must use grants to purchase services and items that are required by the child's Individual Service Plan and unavailable through other funding sources.
Grants may be awarded on a one-time or an ongoing basis and distributed in a lump sum or in monthly payments. For information on how to treat this income, see 0017.15.60 (Family Support Grant Income).
Children eligible for the program are:
● | Under 21 years of age. |
AND | |
● | Certified disabled. |
AND | |
● | Living in their biological or adoptive family home. |
OR | |
● | Residing in a regional treatment center, Intermediate Care Facility for Persons with Developmental Disabilities (ICF/DD), or other licensed residential service or nursing facility, and would return to their family home if a grant were awarded. |
Families who are receiving Home and Community-Based Services under the following Waiver programs may be eligible to receive Family Support Grant funds:
● | Community Alternative Care (CAC). |
● | Community Alternatives for Disabled Individuals (CADI). |
● | Traumatic Brain Injury (TBI). |
Families who are receiving Home and Community-Based Services under the Developmental Disabilities (DD) Waiver are NOT eligible for a Family Support Grant.
A family with more than 1 child with a disability may apply for a grant for each eligible child.
