GRH BASIS - TRANSITION FROM RESIDENTIAL TREATMENT

GRH:
People have a basis of eligibility when residential treatment staff verify they have housing instability when exiting residential behavioral health treatment.

This basis must be verified by residential behavioral health treatment staff in Section 5 (Transition from Residential Treatment to Minnesota Housing Support Program) of the Professional Statement of Need (DHS-7122) (PDF).

The transition from residential treatment basis of eligibility starts on the day the person moves into a setting to receive GRH and continues for the rest of that month through the next 2 full calendar months.

Eligibility under this basis ends at the end of the 3rd benefit month or when the person leaves the setting.imageimageimage
