MnCHOICES assessment training instructions

Intended audience: Agency mentors and staff who work with the process of intake/referral and MnCHOICES assessment and assessment summary (i.e., CSP).

Mentors coordinate training and all MnCHOICES training zone access for all agency staff, providing each staff member with the training email, which includes the MnCHOICES training zone URL, their user name and password and the link to this document.

  • DHS recommends all agency staff take training after mentors provide them MTZ access.
  • DHS recommends all agency staff take the training modules as indicated below.
  • DHS expects agency staff and the mentor to use the resources available them to answer their questions before they submit questions on the MnCHOICES Help Desk Form. We will refer mentors back to training materials as needed.
  • DHS expects mentors to retain and distribute lead agency user names and passwords.

MnCHOICES: Access and navigation, course MNCH900

Approximate time to complete: 4 hours.
Who should take this course: Agency staff (other than assessors)
In this course you will learn: Basic accerss and navigation. Modules include MnCHOICES Training Zone (MTZ) exercises and activities that assist staff knowledge and practice in the application. Exercises build on each other giving staff a hands on knowledge of how to use the application to complete their work. Access and navigation modules need to be taken in order.

  • Introduction and logging into MnCHOICES (15 minutes)
  • My dashboard (15 minutes)
  • Navigation header (15 minutes)
  • Person record (30 minutes)
  • Assignments and transfers (30 minutes)
  • Forms (30 minutes).

Who should take this course: Agency staff (other than assessors)
In this course you will learn: The structure of the MnCHOICES assessment content, layout of items and important information about purpose, common terms and how the assessment content is affected by the person’s age.

  • Introduction to the MnCHOICES assessment: 0.75 CLU (45 minutes)
  • Initiate assessment: 0.5 CLU (30 minutes) - Loaded correct module to TrainLink on 9/21/2021
  • Community living: 0.75 CLU (45 minutes)
  • Daily living: 0.75 CLU (45 minutes)
  • Wellbeing - part 1: 0.75 CLU (45 minutes)
  • Wellbeing - part 2: 0.75 CLU (45 minutes)
  • Using the Mini-cog in a MnCHOICES assessment: 0.25 CLU (15 minutes)
  • Staying healthy: 0.75 CLU (45 minutes)
  • Self determination: 0.50 CLU (30 minutes)
  • Assessment results and MMIS screening documents: 0.75 CLU (45 minutes)
  • Assessment summary: 0.50 CLU (30 minutes)
  • Program acknowledgement and signatures: 0.5 CLU (30 minutes).
