
Fee-for-Service County-Subcontracted Targeted Case Management Rates – Effective January 1, 2026 – December 31, 2026
Counties must use the default rate for each targeted case management (TCM) service area unless the county requests a rate exception from DHS. The rate exception standards and rate exception form can be found on the case management redesign webpage in the “Implementation information for the statewide fee-for-service county-subcontracted targeted case management rate methodology” section.
