Assisted Living Report Card updates
On Feb. 12, 2026, DHS published new ratings and corrected previously published ratings for its Assisted Living Report Card.

Corrected ratings
We corrected the resident health, safety and staffing ratings for 268 assisted living settings (refer to Assisted Living Report Card ratings corrections [PDF]) after we identified errors in data previously provided by MDH and the University of Minnesota (UMN) as part of our ongoing quality assurance efforts.
MDH collected the data, and UMN used the data to calculate ratings for the Assisted Living Report Card. The corrections are due to errors in previously published ratings and do not reflect provider performance.
MDH and UMN found several causes for the errors, including incorrect data entries and calculation mistakes across different datasets and survey years. They partnered with DHS to publish corrected ratings and resolve the source of the errors.

Next update
DHS plans to publish the next update to the Assisted Living Report Card in April 2026.
