Description

Do you use person-centered practices in the service plans you write and in your everyday work? Would you like to improve them? The DHS disability services division has put together a series of modules to help you grow your person-centered skills and apply them to your work. In this training, you will

  • Integrate person-centered practices into assessor and case manager responsibilities and workflows
  • Develop person-centered support plans, such as the community support plan (CSP) and the coordinated services and supports plans (CSSP)
  • Support people so they can lead lives that are meaningful to them.
  • Understand the criteria used by the lead agency review team to evaluate a person centered support plan.

This training complements the “Person-centered thinking” and “Person-centered planning” trainings. The material in this course is the same as the “Using person-centered practices in support planning” workshops offered in 2017-2018.

Learning objectives

  • Understand the term “person-centered” and why Minnesota requires us all to use person-centered practices and philosophies.
  • Understand the value of person-centered practices in your work and what is needed to support your person-centered practice.
  • Be familiar with the Person-centered, Informed Choice, and Transition Protocol.
  • Know who this protocol applies to and understand your role and responsibilities in the delivery of person-centered services.
  • Identify the 12 high-impact protocol items that help us focus our person-centered practices during support planning.
  • Know which person-centered practices are monitored by the lead agency review.

  • Managers and supervisors of the above
