Background

Crisis respite services are specialized services that provide short-term care and intervention to a person. For more information, refer to Crisis respite as a DD Waiver service

Service initiation

To receive crisis respite only as a DD Waiver service option, the person must have an Individual Service Plan (ISP) that documents the:

  • · Amount
  • · Frequency of crisis-respite services
  • · Type.
  • A full team screening (01) or an annual review (02) document must be approved within 30 calendar days prior to the person receiving crisis respite services. The full-team screening or the annual review screening can be completed the day in which crisis services are initiated.

If an annual review document is used to initiate crisis respite services, then a full-team screening must occur within 21 calendar days of service initiation to determine level of care. If crisis services end prior to the 21 calendar days, a full-team screening must be completed before service termination.

When accessing the DD Waiver for crisis respite as an only service, case management cannot be paid for with waiver dollars.

    Service termination

When the crisis respite service is no longer needed, the ISP must reflect this change.

When a person is exiting the DD Waiver after receiving crisis respite only services, complete an action type (05) waiver out document.

When the person is remaining on the DD Waiver, complete an action type (03) service change document to further access other waiver services.

    MMIS

Screening documents submitted to MMIS other than full-team screenings are not automatically reviewed by DSD. All screening documents submitted to MMIS are subject to MMIS system edits. For more information, refer to DD Screening Document Edits (PDF).
