Definitions

Moving Home Minnesota (MHM) home and community-based services (HCBS) coordinator: Professional who helps with intake and confirms a person’s eligibility to participate in MHM.

Lead agency: County, tribal nation or managed care organization (MCO).

Licensed alcohol and drug counselor (LADC): A person who holds a valid license issued under Minn. Stat. §148F.01 to provide alcohol and drug counseling.

Process

To enroll an eligible person in MHM, anyone can complete MHM Intake Form, DHS-5032.

The Senior LinkAge Line (800-333-2433), Disability Hub MN (866-333-2466) or the person’s county or tribal human services agency can help with enrollment in MHM.

Upon receipt of the intake form, an MHM HCBS coordinator will confirm the person’s eligibility. Then, they will work with the person’s lead agency or LADC to facilitate the assignment of an MHM transition coordinator and begin the transition process.
