Day program facility costs

This section of the cost report collects information about each facility the provider organization used, at least in part, for DWRS day programs only, including:

  • · The location of the facility.
  • · Costs related to the facility.
  • · Whether the organization owns or leases the facility.
  • Do not include any facility costs of service provision on this section, including residential facilities or administrative-only facilities. Home and community-based service (HCBS) waivers cannot pay for residential facilities and, therefore, should not be included anywhere in the cost report. For more information, refer to the “what to include section on the home page.

    The cost report preparer should report administrative facilities or administrative portions of facilities that are not for the provision of day services on the general and administrative costs and cost allocation section.

    Facility information

    For each day program facility that is owned, leased or controlled by the organization, provide all the following information:

  • · Street address.
  • · City.
  • · County.
  • · Zip code.
  • · Assessed square footage.
  • Use the drop-down menu under the “own/lease” header to select whether the facility is owned (including facilities paid for under a financed mortgage) or leased/rented.

    Facility costs

    For each facility, use the listed cost categories to provide the total actual expenses during the reporting period.

    In some cases, the organization may be under a lease agreement that includes a “common area maintenance” provision, which means the organization cannot split the cost of the lease/rent from other services paid for through the common area maintenance provision (e.g., building and grounds maintenance). In this situation, the cost report preparer may report all lease/rental and costs funded through the common area maintenance provision under the “mortgage, lease, rent cost” category.

    Describe the nature of “other facility costs”

    Describe the expenses categorized as “other facility costs.” Include enough detail so a person unfamiliar with the organization can understand these costs.
