Overview

Case managers work with people to:

  • · Understand their needs.
  • · Make plans with them to help them meet their needs and dreams.
  • · Connect them with services.
  • · Keep track of their progress.
  • · Follow up with others who are also helping them.
  • Case management types

    DHS administers multiple forms of case management:

    1. Alternative Care (AC) conversion case management

    2. Care coordination

    3. Developmental disabilities (Rule 185)

    5. Relocation service coordination targeted case management (RSC-TCM)

    6. Vulnerable adult/developmental disability targeted case management (VA/DD-TCM)

    7. Waiver, AC and Essential Community Supports (ECS) case management

    8. Other case management:

  • · Child welfare targeted case management (CW-TCM).
  • · Adult mental health targeted case management (AMH-TCM) and children's mental health targeted case management (CMH-TCM).
