Institution for Mental Disease (IMD) Enrollment Criteria and Forms

Minnesota Health Care Programs (MHCP) does not reimburse facilities enrolled only as an Institutions for Mental Disease (IMDs) directly. This provider type is instead enrolled to allow counties to code MHCP members’ living arrangements in the Medicaid Management Information System (MMIS).
An IMD must be certified and licensed by the Minnesota Department of Health or the health department in the state in which it is located.
