Recovery Community Organization (RCO) Enrollment and Forms

A Recovery Community Organization (RCO) is an independent, nonprofit organization led and governed by local community representatives that have a lived experience of substance use. An RCO that is an eligible provider of peer recovery support services can be reimbursed by Minnesota Health Care Programs (MHCP) for providing recovery peer services.

How to Enroll

RCOs who want to enroll with MHCP can use the online MPSE portal or submit forms by fax.

Online MPSE Portal

Register to access the Minnesota Provider Screening and Enrollment (MPSE) Portal and complete your enrollment online using the MPSE portal. Upload the following in the MPSE portal:

  • · Fee-for-Service (FFS) Only or FFS and Managed Care Organization In-Network Provider Agreement (DHS-4138) (PDF) or the MCO In-Network Provider Agreement (DHS-8355) (PDF)
  • · Behavioral Health Fund (BHF) Provider Assurance Statement (DHS-3491) (PDF)
  • · A copy of the certification from the Minnesota Alliance of Recovery Community Organizations 
  • · Proof of general liability insurance in the amount of $2 million for each occurrence. (Certificate holder on the liability certificate must be: DHS Provider Eligibility and Compliance, P.O. Box 64987, St. Paul, MN 55164)
  • · Proof of employee dishonesty bond in the amount of $10,000 if the provider has custody or control of money or property belonging to a client
  • Submit Forms via Fax

    Complete the following documents required to enroll and fax your materials to MHCP Provider Eligibility and Compliance at 651-431-7462.

  • · Organization – Provider Enrollment Application (DHS-4016A) (PDF)
  • · Disclosure of Ownership and Control Interest (DHS-5259) (PDF)
  • · A copy of the certification from the Minnesota Alliance of Recovery Community Organizations
  • · Proof of general liability insurance in the amount of $2 million for each occurrence. (Certificate holder on the liability certificate must be: DHS Provider Eligibility and Compliance, P.O. Box 64987, St. Paul, MN 55164)
  • · Proof of employee dishonesty bond in the amount of $10,000 if the provider has custody or control of money or property belonging to a client
  • RCOs must pay the application fee along with completing enrollment using the MPSE portal or by faxing the completed forms.

    Review and keep a copy of the MHCP Data Privacy Notice (DHS-6287) (PDF).

    We process forms in order of date received. Whether enrolling using the MPSE portal or by fax, allow 30 days for processing. If we need more information to complete your enrollment, we will send a request for more information letter via the U.S. Postal Service (or your MN–ITS mailbox, if you have an account) explaining what you need to do to complete your enrollment.

    Filing an Appeal to the Minnesota Department of Human Services (DHS)

    A recovery community organization that disagrees with a certification determination and believes it meets the requirements as defined in Minnesota Statutes, 254B.05, subdivision 1, paragraph (d) may appeal the determination under Minnesota Statutes, 256.045, subdivision 3, paragraph (a), clause (14), for reconsideration as an eligible vendor. If the human services judge determines that the recovery community organization meets the requirements of a recovery community organization, the RCO is an eligible vendor of peer recovery support services for up to two years from the date of the determination. After two years, the recovery community organization must apply for certification with the Minnesota Alliance for Recovery Community Organizations to continue to be an eligible vendor of peer recovery support services. Refer to the Appeals webpage and follow the process to make an appeal.

    Refer to the Substance Use Disorder (SUD) Services Enrollment Criteria and Forms enrollment section of the MHCP Provider Manual for information about enrolling to provide SUD services other than peer support.

    Revalidation

    MHCP must follow the Centers for Medicare & Medicaid Services’ final federal provider screening regulations.

    MHCP will notify you when you are due for revalidation. When you are notified, you must submit a revalidation request through the MPSE portal or fax all required forms and documents listed in the How to Enroll section to DHS.

    We recommend using the MPSE portal and following the Revalidation Instructions in the MPSE user manual to revalidate your record. Log in to your MN–ITS account on the MN–ITS: Home webpage and choose the Minnesota Provider Screening and Enrollment (MPSE) portal link. If you have not registered your MN–ITS account, your login information is on your original Welcome Letter. Call the MHCP Provider Resource Center at 651-431-2700 or 800-366-5411 if you do not have your Welcome Letter.

    Refer to Revalidation in Provider Screening Requirements under Provider Basics in the MHCP Provider Manual for important information, including timelines, about the revalidation process and impacts to your enrollment. Providers who do not meet the revalidation timelines will have their enrollment terminated.

    Reporting Changes

    Report any changes made on a provider record to MHCP Provider Eligibility and Compliance. Refer to Changes to Enrollment in this manual for details.
