ICS survey

DHS seeks information to better understand the availability, accessibility and quality of ICS services in Minnesota. We want survey input from:

  • · Waiver case managers.
  • · Case management supervisors/managers.
  • · Lead agency leadership (e.g., directors).

    • Survey details

    Use the following information to complete the survey:

  • · Survey available: March 11, 2025.
  • · Completion deadline: March 25, 2025.
  • · How to participate: To complete the survey, refer to the ICS services survey.
  • · Instructions: Complete the survey once. The survey will take 5-20 minutes to complete, depending on your role and responses.
