Overview

Lead agencies can use these resources for help to administer Disability Services Division (DSD) home and community-based services, programs and waivers.

Policy

If you have a general policy question, first look in the CBSM. If you can't find your answer, refer to CBSM – DSD policy help.

Case consultation

If you have a question about a specific person who receives services, submit the question using the DSD Contact Form. For more information, refer to CBSM – DSD Response Center.

MMIS service agreements and screening documents
