ICS providers must report service terminations and temporary service suspensions to DHS

As of July 1, 2026, ICS providers must use the following forms to report service terminations and suspensions to DHS:

  • · Notice of Temporary Service Suspension, DHS-2828C (PDF).
  • · Notice of Service Termination, DHS-2828D (PDF).
  • Each form contains instructions on when and how to notify DHS.

    Reason for change

    The 2026 Minnesota Legislature amended Minn. Stat §245D.10, subd. 3 and Minn. Stat. §245D.10, subd. 3(a) to require ICS providers to report service terminations and temporary service suspensions to DHS.

    DHS actions

    DHS is working to:

  • · Update the forms to include a place to indicate that the termination and/or temporary service suspension is for ICS. We will send another eList announcement when we finish the form updates.
  • · Update CBSM ICS.
  • Provider actions

    Until DHS updates the forms, providers must indicate that the termination and/or temporary service suspension is for ICS in the text box on each form.
