Housing Stabilization Services Enrollment Criteria and Forms

The Housing Stabilization Services (HSS) program ended at 11:59 p.m. on Oct. 31, 2025. Minnesota Health Care Programs will not enroll or reimburse providers for any HSS service provided after Oct. 31, 2025.

  • Minnesota Health Care Programs (MHCP) requires providers to enroll for the services they provide to MHCP members to receive reimbursement. Follow the instructions under How to Enroll to enroll as a Housing Stabilization Services provider. You must also:

  • · Complete the annual Minnesota Department of Human Services’ Vulnerable Adults Mandated Training, which includes training on vulnerable adult law.
  • · Pass a criminal background check.
  • · Be knowledgeable of local housing resources.
  • · Housing Consultation services providers must complete the mandated Housing Consultant training on TrainLink. Search TrainLink’s course catalog to find the training. You will need a unique key to take this training. Complete the New User Form on TrainLink's Training News and Information webpage if you need a unique key.
  • · Housing Transition and Housing Sustaining services providers must complete the mandated Transition & Sustaining training on TrainLink. Search TrainLink’s course catalog to find the training. You will need a unique key to take this training.
  • · All staff working directly with Housing Stabilization Services recipients must complete the Housing Consultant, Transition & Housing Sustaining within 30 days of employment start date.

  • How to Enroll

    Pay the application fee if you use a Federal Employer Identification Number (FEIN) or attach proof that you paid the fee to either Medicare or another state. The payment must be for the same location and to provide the same services. Providers can submit their fees online through the MHCP Provider Screening Fee Collections System. You must pay the fee before applying to MHCP. Note: Housing Stabilization Services providers who own or manage multiple locations where services will be provided must enroll each location separately and pay an application fee for each location. Refer to the MHCP Provider Screening Requirements for information on application fees.

    Providers who wish to enroll with MHCP can do so in one of the following two ways.

    Online MPSE Portal

    Register to access the Minnesota Provider Screening and Enrollment (MPSE) portal and complete your enrollment online using the MPSE portal. If enrolling through the MPSE portal, you will be required to upload the following:

  • · Home and Community-Based Settings Applicant Assurance Statement (DHS-7618) (PDF)
  • · Completed, signed and initialed copy of the Fee-for-Service (FFS) only or FFS and Managed Care Organization In-Network Provider Agreement (DHS-4138) (PDF) or MCO In-Network Provider Agreement (DHS-8355) (PDF)
  • · Request for Licensing Agency ID Number (DHS-3891) (PDF), use this form to submit background studies on all owners and managers. This form must be uploaded separately into the “Notes” area of the MPSE portal if enrolling through the portal.
  • · Applicable assurance statements to the services you plan to provide:

    Submit Forms via Fax

    Complete the following documents and fax your materials to MHCP Provider Eligibility and Compliance at 651-431-7493.

  • · Home and Community-Based Settings Applicant Assurance Statement (DHS-7618) (PDF)
  • · Request for Licensing Agency ID Number (DHS-3891) (PDF), use this form to submit background studies on all owners and managers.
  • · EFT Vendor Number Notification (DHS-3725) (PDF), to be paid through electronic funds transfer (EFT) (optional), refer to Electronic funds transfer (EFT) or direct deposit under Enrollment with Minnesota Health Care Programs (MHCP) in the MHCP Provider Manual.
  • · Applicable assurance statements to the services you plan to provide:

  • Review and retain a copy of the MHCP Data Privacy Notice (DHS-6287) (PDF) for your records.

    We process forms in order of date received. Whether enrolling using the MPSE portal or by fax, allow 30 days for processing. If we need more information to complete your enrollment, we will send a request for more information letter via U.S. Postal Service (or your MN–ITS mailbox, if you have an account) explaining what you need to do to complete your enrollment.

    Revalidation

    MHCP is required to follow the Centers for Medicare & Medicaid Services (CMS) final federal provider screening regulations.

    MHCP will notify you when you are due for revalidation. When you are notified, you must submit a revalidation request through the MPSE portal or fax all required forms and documents listed in the How to Enroll section to DHS.

    We recommend using the MPSE portal and following the Revalidation Instructions in the MPSE user manual to revalidate your record. Log in to your MN–ITS account on the MN–ITS: Home webpage and choose the Minnesota Provider Screening and Enrollment (MPSE) portal link. If you have not registered your MN–ITS account, your login information is on your original Welcome Letter. Call the MHCP Provider Resource Center at 651-431-2700 or 800-366-5411 if you do not have your Welcome Letter.

    Refer to Revalidation in Provider Screening Requirements under Provider Basics in the MHCP Provider Manual for important information, including timelines, about the revalidation process and impacts to your enrollment. Providers who do not meet the revalidation timelines will have their enrollment terminated.

    Risk Level

    All new enrollments, reenrollments, and ownership change requests submitted on or after June 1, 2025, will be screened at the high-risk level. These requests will also be subject to unannounced site visits and fingerprint background checks for all direct and indirect owners with 5% or more ownership. Providers undergoing revalidation on or after June 1, 2025, will also be screened at the higher risk level as part of the revalidation process according to Code of Federal Regulations, title 42, section 455.450.

    Adding Services to existing Housing Stabilization Services Enrollment Record

    To add Housing Stabilization Services to your current Housing Stabilization Services enrollment record, complete and submit the following forms via the MPSE portal or fax to MHCP Provider Eligibility and Compliance at 651-431-7493:

  • Reporting Changes

    The provider must notify MHCP any time a change occurs. Refer to Changes to Enrollment in the Enrollment with Minnesota Health Care Programs (MHCP) section of the MHCP Provider Manual for details.

    MHCP will process the change information in date order received and will notify the agency if any more documentation is necessary to continue or maintain enrollment with MHCP in relation to the changes.
