Health care programs are funded by state government or federal government (or a combination of both). Both state-funded and federally funded health care programs have specific technical or procedural requirements for program eligibility. A person who does not meet program requirements is not eligible for that program.

People may request health care coverage by telephone, in person or in writing. Each person must complete an application and submit all required information to determine if they qualify for a health care program.
